Kidney stones form when there is a high concentration of certain minerals or salts in the urine. A lack of fluids to dilute these substances can lead to crystallization. Over time, these crystals can clump together and grow into stones. They can vary in size, ranging from a grain of sand to a golf ball. These stones can develop in one or both kidneys and can travel through the urinary tract. While small kidney stones can pass on their own without causing symptoms, larger stones can cause intense pain and may require medical intervention.
Causes
- Not drinking enough water. When you're dehydrated, your urine becomes more concentrated with minerals, increasing the chance of stone formation.
- Consuming foods high in oxalates (such as spinach, chocolate, and nuts), sodium, and animal proteins can contribute to stone formation.
- Conditions like gout, hyperparathyroidism, and chronic urinary tract infections (UTIs) can increase your risk.
- If someone in your family has had kidney stones, you are more likely to develop them too.
- Being overweight can change the acid levels in your urine, leading to stone formation.
Types
- Calcium stones are the most common type, often formed from calcium oxalate or calcium phosphate. Calcium oxalate stones are more frequent and form when the levels of calcium and oxalate in the urine are too high.
- Struvite stones can form after a urinary tract infection and can grow quickly. They are composed of magnesium, ammonium, and phosphate.
- Uric acid stones develop when the urine is too acidic. They can occur in people who eat a diet high in purines or who have conditions like gout.
- Cystine stones are rare and occur in people with a hereditary condition called cystinuria, which affects the way the kidneys process certain amino acids.
Symptoms
Kidney stones can show symptoms like:
- Severe pain: Pain is usually felt in the lower back, side, or abdomen and can come in waves. It is often described as one of the most intense types of pain.
- Blood in urine (hematuria): This can cause the urine to appear pink, red, or brown.
- Frequent or painful urination: You may feel the need to urinate more often or experience burning sensations when doing so.
- Nausea and vomiting
- Cloudy or foul-smelling urine: This can indicate an infection, which sometimes accompanies kidney stones.
- Fever and chills: These symptoms may be a sign of an infection and require immediate medical attention.
Diagnosis
Your doctor will likely perform several tests to confirm the diagnosis, including:
- Urinalysis: A urine test can detect high levels of certain minerals and reveal signs of infection or blood in the urine.
- Imaging tests: An X-ray, ultrasound, or CT scan can help locate the stone and determine its size and location.
- Blood tests: These can assess kidney function and identify any imbalances in calcium or uric acid levels.
Treatment Options
The treatment for kidney stones depends on their size, location, and composition, as well as the severity of symptoms.
- For smaller stones, drinking plenty of water (up to 2 to 3 liters a day) can help flush them out of the urinary system. This may be all that’s needed for stones that are less than 5 millimeters in size.
- Over-the-counter medications can help manage pain until the stone passes.
- Some medications can help relax the ureters, making it easier for the stones to pass. Others can dissolve uric acid stones or reduce the amount of stone-forming minerals in the urine.
- Shock wave lithotripsy (SWL) is a non-invasive procedure using sound waves to break the stones into smaller pieces, which can then be passed more easily.
- Ureteroscopy: A small scope is inserted into the urethra and bladder to locate and remove the stone, or to break it into smaller pieces using a laser.
- Surgery: For very large stones that cannot be passed or broken up by other methods, surgery may be necessary.
Prevention
There are several steps you can take to reduce your risk:
- Drinking plenty of water is one of the best ways to prevent stones. Aim for at least 8–12 cups (2–3 liters) of fluids a day.
- Reducing your intake of sodium and animal proteins can help lower the risk of stone formation.
- While it may seem counterintuitive, eating calcium-rich foods can actually reduce the risk of calcium stone formation. Just be cautious with calcium supplements.
- If you’re prone to calcium oxalate stones, limit foods high in oxalates, such as spinach, beets, and chocolate.
