Heart failure occurs when the heart muscle becomes weakened or stiffened, leading to a reduction in its ability to pump blood. This can result in blood backing up into the lungs, causing shortness of breath, or accumulating in other parts of the body.

Heart failure can be classified into different types based on which part of the heart is affected. 

Left-sided heart failure involves the left ventricle, the heart's main pumping chamber. It can be further divided into:

  • Heart failure with reduced ejection fraction (HFrEF): The left ventricle loses its ability to contract normally, meaning it cannot pump with enough force to push blood into circulation.
  • Heart failure with preserved ejection fraction (HFpEF): The left ventricle becomes stiff and cannot relax properly, leading to inadequate filling with blood.

Right-sided heart failure occurs when the right ventricle fails to pump blood effectively to the lungs. It often results from left-sided heart failure but can also be caused by lung diseases or other conditions affecting the right side of the heart.

Causes

Heart failure is usually the end result of various cardiovascular diseases and conditions. Some common causes include:

  • Coronary artery disease (CAD): The narrowing of the coronary arteries due to plaque buildup reduces blood flow to the heart muscle, leading to damage over time.
  • High blood pressure (hypertension): Persistently elevated blood pressure forces the heart to work harder, causing the heart muscle to thicken and eventually weaken.
  • Cardiomyopathy: This is a disease of the heart muscle that can be inherited or caused by other conditions, such as infections, alcohol abuse, or drug use. It leads to an enlarged or stiffened heart, reducing its pumping efficiency.
  • Valvular heart disease: Dysfunctional heart valves, which control blood flow in and out of the heart chambers, can lead to heart failure.
  • Myocardial infarction (heart attack): A heart attack occurs when blood flow to a part of the heart muscle is blocked, causing damage that weakens the heart's ability to pump.
  • Diabetes: Uncontrolled diabetes can damage blood vessels and nerves, contributing to the development of heart failure.
  • Chronic lung diseases: COPD can cause right-sided heart failure due to increased pressure in the pulmonary arteries.

Symptoms 

The symptoms of heart failure can vary depending on the severity and type:

  • Shortness of breath (dyspnea) can occur during physical activity, while lying down, or even at rest.
  • The reduced blood flow to muscles and other tissues can cause a persistent feeling of tiredness.
  • Fluid retention can cause swelling (edema) in the legs, ankles, feet, and abdomen.
  • The heart may beat faster or irregularly as it tries to compensate for its reduced pumping ability.
  • Fluid buildup in the lungs can cause a cough that produces white or pink-tinged mucus.
  • Increased need to urinate at night due to the redistribution of fluid in the body when lying down.
  • Reduced blood flow to the brain can impair cognitive function.

Diagnosis 

Key steps include:

  • A healthcare provider will assess symptoms, risk factors, and family history of heart disease. They may also check for signs of fluid buildup, such as swelling in the legs or abdomen.
  • Echocardiogram test provides detailed images of the heart's structure and function, helping to identify the type and severity of heart failure.
  • Electrocardiogram (ECG) records the electrical activity of the heart, detecting irregularities in heart rhythm and signs of heart damage.
  • Blood tests: Certain blood markers, such as B-type natriuretic peptide (BNP), are elevated in heart failure and can help confirm the diagnosis.
  • Chest X-ray test can reveal the size and shape of the heart and check for fluid in the lungs.

Managing Heart Failure

While heart failure is a chronic condition, it can be managed effectively:

  • Medications: 

ACE inhibitors and ARBs relax blood vessels, lower blood pressure, and reduce strain on the heart.

Beta-blockers slow the heart rate and reduce blood pressure, helping the heart pump more efficiently.

Diuretics, which are also known as water pills, help reduce fluid buildup by increasing urine production.

Aldosterone antagonists block the effects of the hormone aldosterone, which can cause fluid retention and heart muscle damage.

  • Lifestyle changes: 

Reducing salt intake and focusing on fresh fruits, vegetables, whole grains, and lean proteins can help manage symptoms.

Exercise can strengthen the heart and improve circulation, though it's important to follow a plan approved by a healthcare provider.

Quitting smoking and limiting alcohol can reduce additional strain on the heart.

  • Surgical interventions:

 In some cases, surgery may be necessary to treat underlying causes of heart failure, such as coronary artery bypass grafting (CABG), valve repair or replacement, or the implantation of a pacemaker or defibrillator.
