- Female Sex: Women are more likely to develop gallstones due to the influence of estrogen, which can increase cholesterol in bile and decrease gallbladder motility.
- Age: Gallstones are more common in people over 40.
- Pregnancy: Pregnancy can lead to increased levels of estrogen and progesterone, which can slow down gallbladder emptying and increase the risk of gallstones.
- Family History: Genetics play a role, and gallstones can run in families.
- Diet: A diet high in fat and cholesterol and low in fiber may increase the risk of gallstones.
- Rapid Weight Loss: Losing weight quickly can cause the liver to secrete more cholesterol into bile, leading to gallstone formation.
Symptoms
Gallstones can be asymptomatic (referred to as “silent” gallstones) and are usually discovered during imaging tests for other conditions. When symptoms do occur, they are collectively known as a gallbladder attack or biliary colic.
- Sudden, intense pain typically occurs in the upper right abdomen and may radiate to the back or right shoulder blade which can last from a few minutes to several hours.
- Bloating, gas, and a feeling of fullness.
- If a gallstone blocks a bile duct, it can cause jaundice.
- If the pain persists or is accompanied by fever, chills, or severe jaundice, it may indicate a serious complication such as cholecystitis (inflammation of the gallbladder) or pancreatitis (inflammation of the pancreas), and medical attention should be sought immediately.
Diagnosis
Gallstones are typically diagnosed through imaging tests, which may include:
- Ultrasound, the most common method, uses sound waves to create images of the gallbladder and can detect the presence of gallstones.
- CT scans provide detailed cross-sectional images of the gallbladder and surrounding organs, helping to identify gallstones and complications.
- HIDA Scan (Cholescintigraphy) can assess the function of the gallbladder and bile ducts, and identify blockages.
- ERCP (Endoscopic Retrograde Cholangiopancreatography) combines endoscopy and X-rays to diagnose and treat conditions of the bile ducts, including gallstones.
Treatment Options
Treatment for gallstones depends on the severity of the symptoms and the risk of complications.
- If gallstones are not causing symptoms, no treatment may be necessary. Regular monitoring may be recommended.
- Oral bile acid pills can dissolve cholesterol gallstones, but this treatment is usually reserved for people who cannot undergo surgery. The process can take months or years and is not effective for pigment stones.
- For symptomatic gallstones is cholecystectomy, the surgical removal of the gallbladder can be done through traditional open surgery or, more commonly, laparoscopically (keyhole surgery), which is minimally invasive and has a quicker recovery time.
- For those who cannot have surgery, non-surgical methods such as shock wave lithotripsy (using shock waves to break up stones) or ERCP may be used to remove gallstones from the bile ducts.
