Coronary artery disease (CAD), also known as coronary heart disease, is one of the most common types of heart disease and a leading cause of death worldwide. CAD occurs when the coronary arteries, which supply blood to the heart muscle, become narrowed or blocked. This can lead to serious health problems, including heart attacks. 

Causes 

The primary cause of CAD is atherosclerosis, a condition characterized by the buildup of plaque inside the coronary arteries. Plaque consists of fat, cholesterol, calcium, and other substances found in the blood. Over time, this buildup can harden and narrow the arteries, reducing blood flow to the heart. Several factors contribute to the development of atherosclerosis and CAD, including:

  • High Cholesterol Levels: Elevated levels of low-density lipoprotein (LDL) cholesterol, often referred to as "bad" cholesterol, can lead to plaque formation in the arteries.
  • High Blood Pressure: Hypertension can damage the arteries and accelerate plaque buildup.
  • Smoking: Tobacco damages the lining of the arteries and promotes atherosclerosis.
  • Diabetes: High blood sugar levels can damage blood vessels and increase the risk of developing CAD.
  • Obesity: Excess body weight is associated with high blood pressure, diabetes, and elevated cholesterol levels, all of which contribute to CAD.
  • Sedentary Lifestyle: Lack of physical activity can lead to weight gain and negatively affect heart health.
  • Family History: A family history of heart disease can increase the risk of developing CAD.
  • Unhealthy Diet: Diets high in saturated fats, trans fats, salt, and sugar can contribute to the development of atherosclerosis.

Symptoms

The symptoms of CAD can vary depending on the severity of the disease. Some people may experience no symptoms, especially in the early stages. However, as the disease progresses, common symptoms may include:

  • Angina is a feeling of pressure or squeezing in the chest. It often occurs during physical activity or stress and may be relieved by rest.
  • Reduced blood flow to the heart can lead to shortness of breath, especially during exertion.
  • A decrease in blood flow can cause fatigue, even during routine activities.
  • A heart attack occurs when a coronary artery becomes completely blocked. Symptoms may include intense chest pain, pain in the arm or jaw, nausea, sweating, and difficulty breathing.

Diagnosis

If CAD is suspected, a healthcare provider may perform several tests to confirm the diagnosis:

  • Electrocardiogram (ECG or EKG): This test measures the electrical activity of the heart and can detect abnormal rhythms or damage.
  • Stress Testing: During a stress test, a patient exercises while their heart rate, blood pressure, and ECG are monitored. This test can reveal problems with blood flow to the heart.
  • Echocardiogram: An echocardiogram uses ultrasound waves to create images of the heart, allowing doctors to assess heart function and structure.
  • Coronary Angiography: This test involves injecting a contrast dye into the coronary arteries and taking X-rays to detect blockages.
  • CT Angiography: A computed tomography (CT) scan can provide detailed images of the coronary arteries and detect blockages.

Treatment 

The treatment of CAD aims to improve blood flow to the heart and reduce the risk of heart attacks. 

Lifestyle Changes

  • Adopting a heart-healthy diet that is low in saturated and trans fats, cholesterol, and sodium can help manage CAD.
  • Regular physical activity can improve cardiovascular health and help maintain a healthy weight.
  • Quitting smoking can significantly reduce the risk of heart disease and improve overall health.
  • Losing excess weight can help control blood pressure, cholesterol levels, and diabetes.

Medications

  • Cholesterol-lowering Drugs: Statins and other medications can help reduce LDL cholesterol levels.
  • Antiplatelet Agents: Antiplatelet drugs can help prevent blood clots and reduce the risk of heart attacks.
  • Beta-blockers: These medications can lower heart rate and blood pressure, reducing the heart's workload.
  • ACE Inhibitors and ARBs: These drugs can help lower blood pressure and protect the heart.

Surgical Procedures

  • Angioplasty and Stenting: During angioplasty, a balloon is used to open a blocked artery, and a stent is inserted to keep it open.
  • Coronary Artery Bypass Grafting (CABG): This surgery involves taking a healthy blood vessel from another part of the body and using it to bypass a blocked coronary artery.

Prevention 

Preventing CAD involves addressing risk factors and adopting a heart-healthy lifestyle.

  • Eating a balanced diet rich in fruits, vegetables, whole grains, and lean proteins.
  • Engaging in regular physical activity, such as brisk walking, cycling, or swimming.
  • Avoiding tobacco use and limiting alcohol consumption.
  • Managing stress through relaxation techniques, meditation, or therapy.
  • Regularly monitoring blood pressure, cholesterol levels, and blood sugar.
